* Now fetches current number of workers from Heroku API before doing scaling calculation
This doubles the number of Heroku API requests made per app.
Normally not a problem, but since Heroku API quotas are per account, accounts which manage many apps may now go over quota.
In this case use two or more Heroku accounts.

For workers with AMQP `prefetch` setting / concurrency>1 , we were overestimating the number of required workers.
We assumed that each new job would add processing time P to the waiting time, instead of P/concurrency amortized.
