# BUG #12: "Consensus on Vapor" - High Confidence, Zero Deliverables
**Date Discovered:** 2025-10-21
**Date Fixed:** 2025-10-21
**Severity:** High (blocks autonomous CFN Loop execution)
**Status:** ✅ FIXED (validate-deliverables.sh implementation)
**Related:** BUG #11 (template enforcement), Phase 1 & 2 (skill-based processing)
## Summary
Agents report high confidence scores (0.85+) and successful completion, but produce **zero actual deliverables**. The orchestrator's deliverable verification step detects phantom git changes but cannot find the expected files, causing it to hang indefinitely.
**Impact:** 3+ hours lost on a single hung CFN Loop iteration. Autonomous sprint execution blocked.
## Root Cause
### What Happened (Sprint 4.1 Execution)
**Timeline:**
1. **08:26 - Loop 3 Start:** backend-dev-1-1 and devops-engineer-1-1 spawned in parallel
2. **09:30 - Loop 3 Complete:**
- backend-dev-1-1: confidence 0.85 [explicit], files: 1
- devops-engineer-1-1: confidence 0.88 [explicit], files: 1
- Average: 0.86 (gate passed ✅)
3. **09:30 - Deliverable Check:** "Verifying implementation artifacts..."
4. **09:30 - 12:30:** **HUNG** - orchestrator blocked for 3+ hours
5. **12:30 - Manual Investigation:** No files created (`git status` empty)
**Expected Deliverables (None Created):**
- `.claude/skills/agent-lifecycle/checkpoint-state.sh` ❌
- `.claude/skills/agent-lifecycle/detect-pause-signal.sh` ❌
- `.claude/skills/agent-lifecycle/SKILL.md` ❌
- Unit tests ❌
**Actual Deliverables:**
- **Zero files** in git status
- **Zero directories** created
- Agents reported "files: 1" but phantom changes only
## Technical Analysis
### Phase 1 & 2 Skill-Based Processing (Working Correctly)

**What the skill saw:**

**Problem:** `git diff` detected changes, but the expected files don't exist.
### Deliverable Verification (Hung Indefinitely)
**Orchestrator Code (Lines ~900-950):**

**Missing Logic:**
1. ✅ Confidence extraction (Phase 1 & 2 working)
2. ❌ **Timeout on deliverable verification**
3. ❌ **Actual file existence check** (not just git diff)
4. ❌ **Failure mode if files missing**
5. ❌ **Iteration retry with specific feedback** ("create the files!")
## Why Agents Reported High Confidence But Produced Nothing
### Hypothesis 1: Agent Misunderstood Task Scope
**Agent Prompt:** "Implement Redis checkpoint state skill"
**What agents may have done:**
- Analyzed existing patterns ✅
- Designed architecture ✅
- Reported confidence in design ✅
- **Did not write actual code** ❌
**Why:** Agent templates don't enforce "use Write tool" - they report confidence on *understanding* not *deliverables*.
### Hypothesis 2: Tool Use Failure (Silent)
**Agent executed Write tool, but:**
- Write failed silently (permission error?)
- File path incorrect (relative vs absolute?)
- Output not captured by git
**Evidence Against:**
- Phase 1 & 2 pattern doesn't force tool use
- Agents can report confidence without tool calls
- No error logs in agent output
### Hypothesis 3: "Consensus on Vapor" Pattern
**Similar to BUG #11 (Product Owner):**
- Agents interpret "implement" as "analyze and report"
- High confidence = "I understand the task"
- Zero deliverables = "I didn't write code, just planned"
**BUG #11 Fix (Product Owner):**

**Why This Didn't Help Loop 3:**
- Product Owner outputs text (decision)
- Loop 3 agents must output **files** (code)
- Text parsing ≠ file creation
## Impact Analysis
### What Works (Phase 1 & 2)
✅ **Confidence extraction:** Multi-pattern parsing, guaranteed scores
✅ **Parallel execution:** All agents run simultaneously
✅ **No race conditions:** Temp files eliminate polling wait
✅ **Skill-based processing:** Orchestrator captures output reliably
### What's Broken (Deliverable Verification)
❌ **No timeout:** Deliverable check can hang indefinitely
❌ **Phantom deliverables:** `git diff` sees changes, files don't exist
❌ **No iteration retry:** Even if detected, no mechanism to wake agents with "create the files!"
❌ **Blocks autonomous execution:** Cannot trust CFN Loop to run unattended
## Comparison: Before vs After Phase 1 & 2

**Verdict:** Phase 1 & 2 solved confidence extraction but **introduced deliverable verification regression**.

## Attempted Fix Patterns
### Pattern 1: BUG #11 Fix (Template Enforcement)
**Problem:** Agent templates can't force tool usage
**Solution:** Orchestrator parses output instead
**Why It Worked:** Product Owner outputs text (parseable)
**Why It Fails Here:** Loop 3 must output files (not text)
### Pattern 2: Phase 1 & 2 (Skill-Based Processing)
**Problem:** Race conditions in confidence extraction
**Solution:** Skill captures output synchronously
**Why It Worked:** Confidence scores are in agent output
**Why It Fails Here:** Deliverables are files (not in output)
### Pattern 3: git diff Tracking (Current)
**Problem:** Agents don't report deliverables explicitly
**Solution:** Track file changes via git diff
**Why It Fails:** Detects phantom changes, can't verify file existence
## Proposed Fixes
### Fix 1: Add Timeout to Deliverable Verification (Quick)
**Effort:** 30 minutes
**Impact:** Prevents infinite hang
**Limitation:** Doesn't solve "vapor" problem, just fails faster

### Fix 3: Tool Use Enforcement in Agent Prompts (Complex)
**Effort:** 2-3 hours
**Impact:** Agents explicitly told to use Write tool
**Limitation:** Still can't force tool usage (BUG #11 limitation)

### Fix 4: Manual Implementation (Bypass CFN Loop)
**Effort:** 4-6 hours
**Impact:** Guaranteed deliverables, no orchestrator
**Trade-off:** Loses autonomous execution, manual work
**Approach:**
1. Main Chat spawns single agent with explicit file creation task
2. Agent uses Write tool directly (no CFN Loop coordination)
3. Main Chat verifies files created before proceeding
4. Repeat for each deliverable
**When to Use:**
- CFN Loop reliability < 50% (current situation)
- Deliverables are critical infrastructure
- Time budget limited (can't afford 3+ hour hangs)
## Decision: Abort Autonomous Execution
**Reasons:**
1. **Phase 1 & 2 Complete** ✅ (skill-based processing working)
2. **BUG #12 Blocks Sprints** ❌ (3+ hour hangs unacceptable)
3. **Fixes Require Testing** (1-3 hours each, may not solve root cause)
4. **Manual Approach Faster** (4-6 hours guaranteed vs uncertain CFN Loop fixes)
**Impact on Phases 3 & 4:**
- **Phase 3 (Template Simplification):** Low priority, can be done manually later
- **Phase 4 (Pause/Resume):** High value, but blocked by BUG #12
- **Alternative:** Manual implementation of pause/resume (bypass CFN Loop)
## Lessons Learned
### What Phase 1 & 2 Taught Us
**Success Pattern (Confidence Extraction):**
- ✅ Skill-based parsing works reliably
- ✅ Multi-pattern fallbacks prevent 0.0 defaults
- ✅ Parallel execution + temp files eliminate races
**Failure Pattern (Deliverable Verification):**
- ❌ `git diff` is unreliable (phantom changes)
- ❌ No timeout = infinite hang risk
- ❌ Cannot force agents to use Write tool
- ❌ High confidence ≠ actual deliverables
### New Strategy Pattern (STRAT-008)
**When to Use CFN Loop:**
- ✅ Task requires consensus validation (multiple perspectives)
- ✅ Iteration likely needed (refinement cycles)
- ✅ Deliverables are text/analysis (not files)
- ✅ Timeout acceptable (1-2 hours max)
**When to Skip CFN Loop:**
- ❌ Task requires file creation (high "vapor" risk)
- ❌ Critical infrastructure (can't afford failures)
- ❌ Tight time budget (no 3+ hour hangs)
- ❌ Single-shot deliverable (no iteration needed)

## Fix Implementation (2025-10-21)
### ✅ Fix #2 Implemented: Explicit File Existence Check
**Implementation:** `.claude/skills/product-owner-decision/validate-deliverables.sh`
**Features:**
1. Accepts `--expected-files` parameter (comma-separated file paths)
2. Smart task detection (keywords: create, build, implement, generate, etc.)
3. Explicit file existence check via `[ ! -f "$file" ]`
4. Returns PASSED/FAILED immediately (no blocking)
5. Stores missing files in Redis for agent feedback
**Orchestrator Integration:** Lines 940-986 in `orchestrate-cfn-loop.sh`
- Calls validation script immediately after Loop 3 completion
- If FAILED: overrides all confidence scores to 0.0 (prevents gate pass)
- Forces iteration retry without Loop 2 validation
- Provides targeted feedback listing missing files
**Testing Status:** ⚠️ Pending integration test with real CFN Loop execution
## Next Steps
### Immediate (Post-Fix)
1. ✅ Fix #2 implemented (explicit file existence check)
2. ⚠️ Integration test with file-creation task
3. ⚠️ Verify iteration retry works correctly
### Short-Term (Validation)
1. Test with Sprint 4.1 deliverables (checkpoint state skill)
2. Verify missing files feedback propagates to agents
3. Confirm confidence override triggers gate failure
4. Re-evaluate autonomous Phases 3 & 4 execution
### Long-Term (Architecture Improvement)
1. Separate "analysis tasks" from "implementation tasks"
2. Different CFN Loop patterns for each:
- **Analysis:** Confidence on understanding (current pattern)
- **Implementation:** Confidence on file creation (now fixed)
3. Adaptive timeout based on task complexity
4. Better agent prompt engineering (explicit tool use guidance)
## Conclusion
BUG #12 reveals a fundamental limitation: **Phase 1 & 2 skill-based processing solves confidence extraction, but deliverable verification remains broken.**
**Impact:**
- ✅ Phase 1 & 2: Confidence extraction working perfectly
- ❌ Phase 3 & 4: Blocked by deliverable verification hang
- ⚠️ CFN Loop: Unreliable for file-creation tasks
**Recommendation:** Use manual implementation for critical deliverables until BUG #12 fixed.
