# BUG #32: Complete Root Cause Analysis
**Status:** Root Cause Identified (3 Layers Deep)
**Severity:** Critical
**Confidence:** 0.98
## Root Cause (3 Layers)
### Layer 1: Wrong Script Path (Fixed)
**Problem:** Coordinator documented to invoke `orchestrate-cfn-loop.sh` (doesn't exist)
**Fix:** Updated documentation to reference `./.claude/skills/cfn-loop-orchestration/orchestrate.sh`
**Status:** ✅ FIXED
### Layer 2: Documentation-Implementation Mismatch
**Problem:** Coordinator documentation says "invoke orchestrator" but agent doesn't do it
**Explanation:** Agent follows steps 1-7 sequentially, never reaches step 8
**Status:** ⏳ Partially addressed (added CLI mode implementation guide)
### Layer 3: Broken Dependencies Block Orchestrator Invocation (ACTUAL BUG)
**Problem:** Coordinator gets stuck on broken agent-discovery skills before reaching orchestrator invocation
**Evidence:**
- Coordinator hits max iterations (10) trying to run agent-discovery
- Agent-discovery has YAML parsing errors (exits with code 1)
- Coordinator never reaches step 8 (invoke orchestrator)
- No orchestrator execution, no CFN Loop
**Why This Happens:**
1. Coordinator starts execution
2. Step 2: Execute agent-discovery → **FAILS** (YAML parsing error)
3. Agent retries agent-discovery → **FAILS** again
4. Agent tries agent-selector → **FAILS** (depends on registry from step 2)
5. Agent continues troubleshooting for 10 iterations
6. Reaches max iterations, exits
7. **Never invokes orchestrator**

**No orchestrator invocation found in logs.**
## Why Previous Fixes Didn't Work
### Fix Attempt 1: Update Script Path
- ✅ Correct path in documentation
- ❌ Doesn't help if orchestrator never invoked
### Fix Attempt 2: Add CLI Mode Implementation Guide
- ✅ Clear bash example showing how to invoke orchestrator
- ❌ Agent follows steps 1-7 first, gets stuck on broken skills
### Fix Attempt 3: Add Mode Detection
- ✅ Clarifies when to use CLI vs Task mode
- ❌ Still doesn't bypass broken agent-discovery
## The Real Problem
**Coordinator's workflow assumes all skills work.** When agent-discovery fails, the entire workflow blocks.
**This is a cascading dependency failure:**
- Step 2 (agent-discovery) fails
- Step 3 (agent-selector) needs Step 2 output
- Agent spends all iterations trying to fix Step 2
- Never reaches Step 8 (orchestrator invocation)
## Solution Options
### Option 1: Fix Agent Discovery Skill (Proper Fix)
**Action:** Debug and fix YAML parsing errors in `.claude/skills/agent-discovery/discover-agents.sh`
**Pros:** Fixes root cause, enables dynamic agent selection
**Cons:** Time-consuming, may have more issues
### Option 2: Bypass Agent Discovery (Pragmatic Fix)
**Action:** Use hardcoded agent defaults for known task types
**Pros:** Immediate fix, unblocks testing
**Cons:** Less flexible, manual agent configuration
### Option 3: Simplify Coordinator (Minimal Fix)
**Action:** Remove agent-discovery dependency, use minimal config, focus on orchestrator invocation
**Pros:** Fastest path to working CFN Loop
**Cons:** Temporary workaround
## Recommended Approach
**Immediate (Option 3):** Simplify coordinator to use hardcoded defaults
**Near-term (Option 2):** Create fallback defaults for common task types
**Long-term (Option 1):** Fix agent-discovery skill properly
