# BUG #32: Complete Investigation & Resolution **Status:** RESOLVED **Date:** 2025-10-24 **Investigation Time:** 3 hours **Root Cause Layers:** 4 (deepest: missing explicit orchestrator invocation instruction) --- ## Executive Summary **Original Symptom:** CFN Loop never completes - only Loop 3 agents spawn, no Loop 2 validators or Product Owner. **Root Cause:** Coordinator agent documentation described CLI mode workflow but lacked explicit instruction to execute the orchestrator invocation step. **Resolution:** Fixed 4 cascading issues in the CLI mode coordination stack + added explicit orchestrator invocation requirement. --- ## Investigation Methodology Used **Task Mode** (step-by-step visibility) to systematically test each component in the CLI mode pipeline: 1. ✅ Task Classification → Agent Discovery → Agent Selector → Orchestrator 2. Found and fixed bugs at each layer 3. Validated fixes in isolation before testing integration --- ## Root Cause Layers (Deepest to Surface) ### Layer 4: Missing Orchestrator Invocation Instruction (DEEPEST) **Problem:** Coordinator documentation showed "invoke orchestrator" as step 8, but agent interpreted this as aspirational rather than executable. **Evidence:** - Coordinator completed steps 1-7 successfully - Hit max iterations (10) without calling orchestrate.sh - No Redis keys for orchestration activity **Fix:** Added explicit, non-optional instruction in coordinator documentation: ```markdown **CRITICAL CLI Mode Requirement:** After completing steps 1-7, you MUST invoke the orchestrator using Bash tool. **CLI Mode Implementation (REQUIRED FINAL STEP):** [Complete bash code example showing exact command to run] **This step is NOT optional. If a step requires action (running a script), provide explicit executable code with clear "MUST DO" language.
