Blameless Culture
# Principle
**Failures are inevitable. They are the single greatest learning opportunity.** Study them, don't punish them.
# The Foundation
A culture that punishes failure incentivizes engineers to:

- ❌ Avoid risks
- ❌ Blame others
- ❌ Cover up problems
A blameless culture incentivizes engineers to:
- ✅ Admit mistakes immediately
- ✅ Take calculated risks
- ✅ Take ownership
- ✅ Learn from failures
# Core Assumption
**Every person involved acted with the best intentions based on the information they had at the time.**
This is the required baseline for all postmortems and incident reviews.
# Blameless ≠ Accountability-Free
**Blameless does not mean:**
- No consequences
- No accountability
- No performance management
- Tolerating negligence
**Blameless means:**
- Focus on systems, not individuals
- Learn from failure
- Improve processes
- Prevent recurrence
# The Postmortem Process
## Triggers
A blameless postmortem is **mandatory** for:
1. **User-visible downtime** or degradation
2. **Data loss** of any kind
3. **Security incident** or breach
4. **Manual on-call intervention** (emergency rollback, manual DB change)
5. **Monitoring failure** (incident discovered manually, not by alerts)
## Timeline
- **Within 24 hours:** Schedule postmortem meeting
- **Within 48 hours:** Complete postmortem document
- **Within 1 week:** All action items assigned and prioritized

- Incident Commander
- Engineers involved
- On-call engineer
- Engineering Manager/CTO

5. **Why?** We prioritized speed over reliability

# Psychological Safety
## Creating Safety
**Do:**
- ✅ Thank people for raising issues
- ✅ Celebrate learning from failures
- ✅ Share your own mistakes publicly
- ✅ Focus on "how do we prevent this?"
- ✅ Assume good intentions
**Don't:**
- ❌ Ask "who did this?"
- ❌ Punish people for mistakes
- ❌ Shame or embarrass
- ❌ Focus on "why did you do this?"
- ❌ Assume malice or incompetence

"What testing would have caught this? How do we make that automatic?"

## Accountability (Good)
**Definition:** Taking ownership and responsibility for outcomes
**Example:**

## Blame (Bad)
**Definition:** Assigning fault to punish

# When Accountability Matters
Blameless culture does NOT mean:

If the same person makes the same mistake repeatedly:
- **First time:** Learning opportunity, blameless

## Negligence
If someone deliberately ignores safety procedures:
